Telehealth Consent

Effective date: [DATE OF LAUNCH]

  1. Acceptance of terms

By accessing aerismd.com or using AerisMD services, you agree to these Terms of Service. If you do not agree, do not use our services.

  1. Description of services

AerisMD provides telehealth dermatology services in states where Dr. Soheil Simzar holds an active medical license. Services may include online consultations, prescription evaluation, and ongoing dermatologic care. AerisMD does not provide emergency medical care and is not a substitute for in-person evaluation when clinically indicated.

  1. Eligibility

To use our services you must:

  • Be at least 18 years old (or have parental consent if a minor patient)

  • Be physically located in a state where Dr. Simzar is licensed at the time of consultation

  • Provide accurate, complete, and current information

  • Not have any condition that requires emergency or in-person care

  1. The doctor-patient relationship

A physician-patient relationship is established only when Dr. Simzar accepts your case after reviewing your consultation. Submitting a consultation does not by itself create a treatment relationship.

Not for emergencies

AerisMD is not for medical emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room. Do not use AerisMD to communicate emergency concerns.

  1. Payment and refunds

  • Fees are charged at the time of service

  • We do not accept insurance; AerisMD is a cash-pay practice

  • Subscription memberships renew automatically until canceled

  • Refund policy: [LAWYER TO ADVISE — telehealth refund policies vary by state. Common practice: refunds available before physician review begins; non-refundable once consultation has been completed.]

  1. Prescriptions

  • Prescriptions are issued at the discretion of the treating physician based on clinical judgment

  • Submitting a consultation does not guarantee a prescription

  • Prescriptions may not be appropriate for all patients

  • Compounded prescriptions are dispensed by partner pharmacies; AerisMD does not directly dispense medication

  1. User responsibilities

You agree to:

  • Provide truthful, accurate medical information

  • Not misuse the service or provide false information to obtain prescriptions

  • Maintain confidentiality of your account credentials

  • Not share medications with others

  • Notify us promptly of changes to your medical condition or other medications

  1. Intellectual property

All website content, logos, and materials are the property of AerisMD or our licensors and protected by copyright and trademark law. You may not reproduce, distribute, or use them without permission.

  1. Disclaimers and limitation of liability

[LAWYER TO COMPLETE — standard telehealth liability language, including disclaimers regarding the limits of remote care, the patient's responsibility to seek in-person care when indicated, and limitation of damages to the maximum extent permitted by law.]

  1. Dispute resolution

[LAWYER TO COMPLETE — typically includes arbitration clause, governing law (likely California), and venue specification.]

  1. Changes to these terms

We may update these Terms. Continued use after changes constitutes acceptance.
